New Jersey State Registry – Electronic & Mail-In Registration

1. Registering Your PAD with the State.

An individual can choose to register a copy of their psychiatric advance directive with the state.

• • The benefit of registering a plan is that the treating medical professional in a Screening Center, Short Term Care Facility (STCF),or state psychiatric hospital can get a copy of it in a timely manner.
  • The database of PADs at this website is held in a confidential and secure site, protected by the most up-to-date web and database security standards.

  2. Registering Your PAD Electronically:

  This link will redirect you to NJPAD, the online registry operated by the U.S. Advance Care Plan Registry (USACPR): https://www.state.nj.us/humanservices/dmhas/resources/mental/pad/#1

  • • Using this link, you can create a Registrant Account to register your PAD with the state.
    • Your registration is not complete until you upload a document.

    Electronic submission of PADs is preferred; however, individuals may also submit completed hard-copy PADS via mail.

    3. Registering Your PAD via Mail:

    **Mailed in PADS MUST be accompanied by a completed PADS Registration Agreement.**

    • • The Registration Agreement MUST be mailed in with a completed PAD for the PAD to be filed.

      MHANJ Psychiatric Advance Directive Training Program

      The MHANJ offers Three Specialized PAD Trainings:

      • Individuals living with mental health conditions – those seeking information about how a PAD can benefit them, and how to complete it.
      • Family member/friend training – those seeking information about how a PAD might help their loved one, and how to help them complete it.
      • Providers/behavioral health staff – those seeking information about how to help their clients complete a PAD, what their legal requirements are and what the benefits of a PAD are for both the provider and individual.
